Does NOT Integrate with Zapier
I purchased this app specifically for the Zapier integration that was mentioned, only to learn (as I'm in the middle of setting up a Zap), that Formly is not listed as one of the app connections available on Zapier. Then, when reaching out to their "non-tech" AI automated responses Tech Support, I learn that in order to integrate with Zapier, I must send my responses to a Google Sheet. News flash! Not everyone uses Google Sheets, and this was not part of my plan. You really shouldn't be advertising that this integrates with Zapier, when it actually doesn't integrate directly. The ONLY reason that I'm not going to request a refund is because I like the fact that I can use custom domains; otherwise, I'd just stick with FormNX. Highly disappointed with this purchase.

I've been really impressed
I'd been looking for an alternative to QuestionScout, Typeform, and many others that have recurring monthly costs and I've been really happy with Formly so far.
I was able to have a 1:1 onboarding call with one of the team members to get a full walkthrough of the current features and hear what's in store on the roadmap as well. It was extremely helpful as I began implementing surveys myself as I'd been able to see what's possible in the onboarding. I wasn't just able to migrate versions of my current surveys and quizzes but take them to the next level.
I've now been using it for a month and a half - the tool has been working reliably and the layout is very easy to use, even with the logic. I'd definitely recommend this to anyone looking for a great tool for forms, surveys, and quizzes.

Highly Recommend Formly
I needed to create a conditional logic survey and clutched my pearls at the price of Typeform. Decided to give this Formly a try and WOW!!! I'm very impressed.
1. The support is amazing. Ash is very responsive to emails.
2. I scheduled a Zoom, and Ash walked me through my form, helping me correct a few things I had wrong as well as suggesting how to make my form better.
The ability to add a visual chart representing the score is going to have the exact impact I am looking for.
The only suggestion I have is that the progress bar be positioned more visibly than it is during the quiz.

Custom domains not working
I upgraded my account to purchase additional custom domains for my forms.
These stopped working recently. The company admits they are not working. But they don't know why.
I have an entire content strategy that launched and used those broken custom domains. It completely destroyed my campaign.
Support has been useless and slow. I couldn't be more disappointed in Formly.
Save your money and invest in a different form builder.

Apr 22, 2026I’m really sorry for the disruption caused. From our investigation so far, the issue appears to be specific to your domain, as other custom domains are functioning correctly. Because of this, it’s taking a bit longer than expected to pinpoint the exact cause. But we’re actively working on it and will have a solution as soon as possible.
